System of Care

The System of Care model is not a program - it is a philosophy of how care should be delivered.

The System of Care model is an approach that recognizes the importance of family, school and community. It seeks to promote the full potential of youth by addressing their physical, emotional, intellectual, cultural and social needs.It is the foundation for successful Child and Family Teams.

At-risk youth receiving community services should have a cross systems Child and Family Team. Using a strength-based approach, the purpose of the team is to identify formal and informal supports available and to work collaboratively to plan and access these services/ activities. To learn more about the Child and Family team we encourage families and community services to take the Child and Family Team training.
